Tristan Johnson
Tristan Johnson has been a registered learning disability Nurse for 20 years. Recent roles have involved working in a strategic capacity across Norfolk and she is currently the Acute Liaison Nurse at the Norfolk and Norwich University Hospitals Foundation Trust (NNUHFT).
Her key focus is to support primary and acute care, to put systems in place to enable people with learning disabilities to make more informed choices about their health care and to improve their health outcomes.
Tristan is passionate about ensuring that continued improvements are made across the health and social care economy, to ensure equitable health access and outcomes for this vulnerable patient group. She is currently undertaking the Next generation learning disability leadership programme with the National Development Team for Inclusion to support this.
Her recent achievements include working with G.P practices to gain an 89% sign up to the learning disability Direct Enhanced Service. With colleagues in 2007 she won the East of England (EoE) NHS Live innovation award for the Norfolk Health Action plan pathway. In 2010 she was runner up in the EoE innovation awards and was national winner the Nursing Times Chief Nursing Officer and patient pathway awards.
