The call to strengthen care in the home and community is becoming louder. Key to increasing care at home and in the community is innovation and the movement of resources from acute care to the community.
This conference will explore how we can better serve patients, their families and carers in the home through new ways of commissioning, providing, and developing the workforce.
The event will see the launch of the QNI online resource ‘Transition to Community Nursing Practice’ – a resource to support nurses moving to the community setting.
